This is a full-time non-exempt position that is responsible for working with adult individuals in an office and/or community setting. The goal of the Employment/Education Community Health Worker (CHW) is to address individuals Social Determinates of Health (SDoH) needs along with focusing on identifying employment and education needs. The Employment/Education CHW will be responsible for outreaching to sites in the community where individuals who have unmet needs are located and prioritizing addressing barriers by locating and providing resources. Developing relationships with local/private employers to assist in gaining employment and with colleges/universities to assist with education needs for individuals will be prioritized. The Employment/Education CHW will provide essential linkages and follow up services. Primary work hours will be Monday through Friday, 8:30 am to 4:30 pm, with a 1/2-hour lunch (37.5/week) unless otherwise agreed upon.
It is the expectation that employees will adhere to any/all RIHS, county, state, and federal guidelines, regulations, policies, and laws governing The Community Based Care Management Program (CBCM). Employees of the CBCM program will be expected to attend specially designed training programs provided through RIHS, Managed Care Organizations, county, state, and federal entities that are relevant to the CBCM program.
ESSENTIAL DUTIES AND RESPONSIBILITIES

The Employment/Education CHW will provide services to their clients either by phone or in person based on the individual’s needs and preference. When meeting with clients in the community, the Employment/Education CHW will ensure all information is secure and kept private and meetings are in a safe/confidential location. The Employment/Education CHW will formulate relationships with both employment and education stakeholders with the intent to hire individuals will be vital. This person will build connections with local colleges and universities to enable individuals to gain access to grants and loan opportunities. The activities of the Employment/Education CHW will include conducting standardized SDoH screening and interviewing to prioritize and meet the SDoH needs of the client, advocating to empower the individuals they serve, completing referrals for other community services that will assist in improving the client’s quality life and maintaining proper communication with these providers. The employee will maintain documentation of case activity, electronic records, and proper follow-up to ensure the referrals/linkages/warm handoffs were successful and effective. The Employment/Education CHW will be responsible for preparing reports within required timeframes as mandated by federal, state, and/or county entities.
The Employment/Education CHW must have the ability to identify and defuse individuals who are in crisis as a result of not having their SDoH needs met.
The Employment/Education CHW must have extensive knowledge of theories, principles, and techniques related to service provided. They need to have the ability to establish/maintain effective working relationships with clients, associates, and agency personnel. A knowledge of community resources and the capacity to facilitate coordination activities and an ability to prepare clear, concise reports and to communicate effectively, both orally and in writing, is necessary.

Education and/or Experience:
- Bachelor’s degree in Social Science related field preferred and one (1) year of full-time volunteer or paid employment in a social service-related position preferred, or
- A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and equivalent experience working in the Social Services field.
Employee must have strong interpersonal skills, effective oral and written communication skills and the ability to prepare clear, concise reports. They must possess working knowledge of computer software that includes databases, spreadsheets, and word processing for tracking information and preparing reports, letters, and forms. The ability to effectively present information, and respond to questions from clients, families, agency, county, and state representatives is also necessary.
Certificates, Licenses, Registrations:
Valid PA driver’s license, Proof of valid Auto Insurance, ACT 33 (Child Abuse), ACT 34 (Criminal Record) and FBI Clearance. Employee must meet the requirement for becoming a Certified Community Health Worker established by the Pa Certification Board (PCB) within 1 year of being hired. There will be monthly screenings of PA Medicheck List, LEIE and EPLS.Physical/Mental Demands:
While performing the duties of this job, the employee is regularly required to operate a motor vehicle, sit, stand, walk, bend and lift objects of up to 10 pounds. The employee regularly uses a computer, phone, fax, copier and other essential office equipment to carry out daily work assignments.
The Employment/Education CHW will work primarily in the community and will offer services wherever the client location may be; in the client’s home, community center, and place of worship or treatment center. The Employment/Education CHW will conduct screenings, provide resources, problem resolution and informal support network building for individuals served where and when it is needed. The Employment/Education CHW will be required to drive throughout Westmoreland County and its surrounding counties. Exposure to outside weather condition will exist. The noise level in the work environment is usually moderate.
