What are Social Determinants of Health?

 

Social Determinants of Health (SDoH) are non-medical factors that influence health outcomes. These are conditions in the environment in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality of life outcomes and risks.

Social determinants of health that we address consist of the following needs :

  • Childcare
  • Clothing
  • Food
  • Finances
  • Education
  • Employment
  • Healthcare
  • Housing
  • Transportation
  • Utilities

Barriers to SDoH increase the risk factors of an individual’s ability to lead a healthy lifestyle and consequently to retain a stable, health home environment within the community.

Our outreach program targets persons 18 years and older with behavioral health needs in Westmoreland County who are experiencing challenges in daily living due to instability in several areas of life that impact overall health and wellness.  Our Community Health Workers work with participants in assessing their priority needs and develop skills in :

  • Resource identification and acquisition
  • Financial literacy
  • Medication Management
  • Behavioral Health Management
  • Employment : Job search techniques, resume writing, interview preparation, and workplace etiquette.
  • Social Integration: Building social networks, communication skills, and community engagement.
  • Problem-Solving: Identifying challenges and developing strategies to overcome them.
  • Health Management: Understanding healthcare needs and managing personal health effectively.
  • Education: Accessing educational opportunities and improving learning strategies.

 

Who is eligible?

Westmoreland County residents, age 18 and over, who are MA eligible, and have behavioral health needs.
